Our Certified Community Behavioral Health Clinic (CCBHC) provides access to coordinated comprehensive behavioral health and substance use care intended to promote access to integrated behavioral services, including 24/7 crisis response and medication for assisted treatment of Opioid Use Disorder, through use of a behavioral health home model of care. We offer a comprehensive range of services available to people who need care regardless of age, ability to pay and location don’t have to piece together the behavioral health support they need across multiple providers.
The Outreach and Engagement Network Coordinator role is responsible for delivering work in the community, connecting pregnant women and mothers with dependent children to the Program and related services, promoting awareness, reducing barriers, and fostering trust within the community and building community stakeholder relationships and linkages and recruiting community collaborative partners.
Oversees the MOMS Care Coordination Pathway Program for Pregnant Women and Women with Dependent Children, ensuring program goals are met, services are delivered effectively, clinical staff supervision, and community partnerships are maintained.
The Maternal Child Health Nurse (MCH Nurse)/OB Nurse Liaison serves as a critical bridge between prenatal care, medication-assisted (MAT), behavioral health, and MAR recovery services. This role ensures that pregnant women and mothers with dependent children receive integrated, coordinated care throughout pregnancy, postpartum, and early parenting conducts on-site, home visits, and Mobile Clinic visits in the community.
Supports program operations, data collection, reporting, and day-to-day logistics.
The Clinical Therapist provides individual and family therapy for clients between the ages of 18 and above. The Clinical Therapist maintains flexible hours to accommodate client who are employed including evenings and weekends. Engaging significant others in the therapeutic process may be essential to addressing the needs of the client’s therapist should have expertise in group counseling. The Clinical Therapist is responsible for ensuring that multidisciplinary Treatment Plans are developed in a timely manner and that clients with complex needs are firmly linked to appropriate services within TLOC and other community-based resources.
Prepares and maintains a confidential, accurate, organized, and updated case file for each assigned client. Adheres to documentation deadlines as outlined in The Link & Option Center, Inc., 77 Illinois Administrative Code Part 2060 Policy and Procedure Treatment Training Manual.
The Case Manager (CM) is responsible for ensuring continuity of care for TLOC clients who are identified as high-risk for psychiatric hospitalization or detainment and those who are at risk of being Lost-to-care. The CM ensures that clients are monitored consistently for clinical and/or medical engagement, and that their Social Drivers of Health (SDOH) needs are met to ensure that clients remain stable within the community and minimally utilize institutional care. The CMprovides close and consistent tracking of TLOC clients’ engagement in primary care, psychiatry, and other needed services. The CM is the primary point of contact for identified clients and provides overall management of the system of care and client engagement within the system (i.e., reducing barriers to engagement such as transportation, lack of social supports, and access to needed community resources.
